I ordered a OAG, Lodestar x2 and Lacerta (focuslock) last week. So hopefully will solve such issues...!!

I’ll look at the tilt (Rolando says guiding etc was good) as there are lots of attachments in the 1.6 Tak Extender setup. Good point.

Doesn't enter into it. The guiding is spot on, so the mount and guide scope were tracking perfectly.

Your guiding looks perfect. Your imaging camera is drifting with respect to the guider - differential flexure between imaging camera and guide camera. One or the other is loose.

No, it's not polar alignment since your guiding is ok (.4arc sec RMS).
Not enough information about your setup. However, i will take a guess that you have a separate guide scope. Because you are

Since your mount is 2 years old, it is most likely that the worm has worn. Worms will wear in much faster if you do a lot of slewing, and will change
