What exactly is the 'last version' of TSX, version and build #?
If TSX is crashing you should turn on the black box recorder in preferences and set it up to send the crash log to SB.

David, I may not be complete help here but I can make a few comments based on my AP900GTO CP2 and TSX. My chip was the very latest one for the CP2 (can't remember the version, sorry)

With a decent sized scope and mount, it's not unusual for big slews to be several arc-min off, even with the best pointing

I have crossed posted this in the PHD2 forum. A kind soul has plotted my graph in terms of RA position and it looks like there is something loose in the RA axis. I will check the mounting plate etc

Could you post your PHD2 Debug Log and Guide Log from the session in the PHD2 forum. We can analyze the logs and check to see if it is related to the mount or if it is something else.
