What does "Bad X scale" mean as a Status message in APPM Plate solving mode? These do not count as "Bad
Solves." Should I even worry about them?
It means the platesolve succeeded but the image scale was outside of the image scale tolerance you have defined.

It means it was likely a bad plate solve.
